Training and exam
Training: Iowa Code 152C.3 requires a curriculum of at least 600 hours of supervised academic instruction at a board-approved school that requires a high school diploma or equivalent for admission. A board-approved school must be recognized by the board in its state (or a similar licensing board) and not denied, suspended or revoked by NCBTMB. Applicants from schools with fewer than 600 hours, or from schools sanctioned by a national association or government body, need Board review before a license is issued. Foreign-trained applicants need a credential evaluation from IERF, International Credentialing Associates or Josef Silny & Associates.
Exam: Iowa accepts the MBLEx or any NCBTMB exam. Scores must be sent directly from the testing organization to the Board. The passing score is the one set by the testing organization when you took the test. Iowa has no separate state exam.

Keeping your license
Renewal: Licenses renew every two years and expire on the 15th day of your anniversary month (the month the license was issued). Renewal costs $60. You need 16 continuing education hours each two-year period, and at least 8 of them must be in person or through live, real-time interactive instruction; you do not need CE for your first renewal. Up to 8 extra hours can carry over to the next period. If your work involves treating children or dependent adults, you must also complete the state's mandatory reporter training on child and dependent adult abuse. There is a 30-day grace period with a $60 late fee; after that, the license becomes inactive and you may not practice until you reactivate.
Good to know in Iowa
Licensed in another state: You can apply by endorsement if your other state's requirements equal or exceed Iowa's, with transcripts, MBLEx or NCBTMB scores and license verification. If you do not qualify, you may apply by verification under Iowa's general rule, or get a 1-year non-renewable temporary license ($120) while you finish an Iowa-approved program.
Moving from an unlicensed state: If you moved to Iowa from a state that did not license massage therapists, you may be able to use proof of work experience instead of the education requirement under Iowa's work experience rule.
Unlicensed practice penalties: The Board can fine someone who practices without a license, or a business that employs an unlicensed person, up to $1,000 per offense and up to $10,000 for a continued violation.
ID on request: If a peace officer investigating a complaint of illegal services asks, you must show a copy of your valid Iowa massage license and a government-issued ID (such as a driver's license, state ID or passport). Refusing is a serious misdemeanor. Your license must also be displayed at your main practice site.
